Carebridge Server

v1.0.0

A powerful MCP server built with NitroStack

Connection Setup

Add via Cursor Settings UI (Settings > Features > MCP > Add New MCP Server):

{
  "mcpServers": {
    // your other mcp servers
    "carebridge-server": {
      "url": "https://sassy-coders-6a6c7c0d-sassy-coders-srmist.app.nitrocloud.ai/mcp"
    }
  }
}

Connect remote tools directly via Claude's Web UI:

Add custom connector BETA
Connect Claude to your data and tools. Learn more about connectors or get started with pre-built ones.
Advanced settings
Only use connectors from developers you trust. Anthropic does not control which tools developers make available and cannot verify that they will work as intended or that they won't change.

Configure custom tools directly via ChatGPT's Web UI:

New App
PNG only. Best results at 256 x 256 px or larger. Max file size: 10 KB
Custom MCP servers introduce risk. Learn more
OpenAI hasn't reviewed this MCP server. Attackers may attempt to steal your data or trick the model into taking unintended actions, including destroying data.

Add the following configuration block under mcpServers in your Antigravity configuration file (~/.gemini/config/mcp_config.json):

{
  "mcpServers": {
    // your other mcp servers
    "carebridge-server": {
      "serverUrl": "https://sassy-coders-6a6c7c0d-sassy-coders-srmist.app.nitrocloud.ai/mcp"
    }
  }
}

Add the following configuration block to your Codex configuration file (~/.codex/config.toml):

[mcp_servers.carebridge-server]
url = "https://sassy-coders-6a6c7c0d-sassy-coders-srmist.app.nitrocloud.ai/mcp"

Connect directly using the Server-Sent Events endpoint:

Available Tools
analyze_baseline

Compares current health data with the patient's baseline vitals and detects significant changes in sleep, heart rate, activity, and meals.

search_clinical_knowledge

Performs semantic vector and keyword search over medical clinical guidelines, SNOMED-CT triage rules, and evidence-backed protocols stored in MongoDB Atlas.

check_red_flags

Screens user reported symptoms against deterministic medical red-flag safety rules to identify emergency or urgent conditions. This is a deterministic check — the LLM cannot override results.

start_triage

Initiates adaptive triage conversation flow and evaluates urgency category with escalation factors.

get_patient_context

Retrieves patient profile, baseline vitals, recent weekly health state, medical history, and timeline events dynamically by patientId.

analyze_health_history

Analyzes longitudinal lab trends (e.g., 6-month Hemoglobin decline) and connects current symptoms with historical medical context for a patient.

generate_health_timeline

Generates a rich visual health timeline combining longitudinal lab results, guardian vitals events, and today's symptom report.

generate_doctor_brief

Generates a complete, presentation-ready clinician handoff brief including chief complaint, baseline changes, lab trends, medications, and recommended actions.

export_fhir_bundle

Exports raw HL7 FHIR R4 JSON Bundle for a given patient ID, enabling EHR interoperability.

seed_mongodb_database

Triggers seeding or verification of MongoDB Atlas collections (patients, observations, clinical guidelines).

orchestrate_carebridge

Runs the full CAREBRIDGE AI intelligence pipeline from a single user message. Chains: Guardian baseline analysis → patient context → health history intelligence → triage follow-up questions → deterministic red-flag check → urgency classification. Use this when a patient reports a health concern to trigger the complete care navigation workflow.
